Soft callus formation felt at fracture site but still shifting. Left bandage off. Full extension before PT so did a couple stretches and replaced fabric wrap on right wing. No bruising or swelling on right elbow.

Cleaned wound, applied topical lidocaine, closed with 5-0 webcryl in a simple continuous pattern, covered with telfa/tegaderm. Took x-rays: proximal aspect of fracture site healing, but comminuted center of fracture still lacking good callus. Replaced fabric wrap.

O: Removed wing wrap. Still can feel instability at the distal aspect of the fracture site, much more than would expect after 26 days in care. Wing very tight during PT, bird acting painful. Noted that elbow was bruised and swollen. Gave bird 0.16 ml torb IM. Examined wing at fracture site closely - found small piece of dead skin over an open wound along the underside of the dorsal fracture site, removed a small bone fragment as well. Flushed wound with saline, covered with honey/telfa/tegaderm. Placed fabric wrap over wing.

A: Removed dead tissue from underside of fracture - this is likely why fracture is not healing fully. Hopefully now that site has been cleaned and debrided will start to see more healing. Bruising/swelling on elbow likely secondary to wing wrap being too tight, switched to fabric wrap.

P: Change bandage in 3 d with PT. Monitor fabric wrap.

A: Delayed healing of R ulna fracture. Causes for non-union/poor healing would include lack of stability at fracture site, infection - seems to be staying stable with wing wrap, no obvious signs of osteomyelitis on x-rays but there was an open wound at the fracture site originally. Will give more time with diff meds.

P: Switch from clindamycin to Clavamox. Change wrap with PT in 3d, repeat blood this week and x-rays next week.

Gave bird 0.14 ml torb IM, cleaned wound site with chlorhex and flushed with saline, applied lidocaine to the edges of the wound. Wound appears deep, extending to the fracture site. Closed with 4-0 webcryl in a simple continuous pattern. Covered sutures with telfa/tegaderm and replaced figure-8 wrap. Drew blood from the R medial metatarsal vein.

Gave 0.14 cc torb IM, meloxicam, clindamycin PO and 25 cc LRS SQ in the R inguinal. Covered wound on R wing with duoderm gel, telfa/tegaderm and placed figure-8 wrap on R wing.

A: HBC BAOW with head trauma, road rash around the tail/uropygial gland, and fracture to the R ulna. Very comminuted midshaft fractures tend to heal well without surgery, so will start with conservative management. Wound on wing should be assessed further and potentially closed with sutures when bird is more stable.

P: Change wing wrap tomorrow to see if wound on wing can be sutured when bird is more stable. Plan to treat ulna fracture with conservative management unless issues arise. Rx clindamycin, meloxicam, tramadol, ca slurry.
